Here is FORM that you need to sign to go to the spinning gym in Berkeley.

Norcal MTB League Release Form

This is the mandatory release form required by the League in order for team riders to be covered by the League's liability insurance policy. Please print out and sign this FORM.

Parent's Consent and Medical Examination (Form 6)

Automobile Transportation (Form 5)

Medical Authorization/Liability Release (Form 4)

Please print out and sign both pages of this form (page 1 and page 2).

Agreement for Use of Bike and Equipment (Form 3)

All riders must print out and sign both pages of this form (page 1 and page 2), even if they have their own bike and/or equipment. The purpose of this form is to address issues that may arise if you borrow or are given a bike (or equipment) to ride or use during team activities.
